Shwartz Hospice & Palliative Care Agency, we understand the importance of a holistic approach to end-of-life care, which focuses on the needs of the person physically, emotionally and spiritually as well as the impact on his or her family.
The decision to choose our hospice care program is made in collaboration with the individual’s family, physician, and the Shwartz Hospice team of trained professionals.
Our interdisciplinary team develops a plan of care that addresses the unique needs of the person as well as their family. Individuals receive our skilled care in the familiar surroundings of their home, whether that home is a private residence, nursing home, or assisted living facility. The hospice team works in conjunction with the individual’s attending physician and personal surroundings in order to provide optimal comfort and support.
